Terms of use

These are the terms and conditions of use set forth by the leaders of “Canadian Thyroid Patients Campaign.”

Information and advocacy purposes only

The information contained within this website and our social media outlets is for general information purposes and system-wide advocacy purposes.

Our campaign seeks to achieve evidence-based improvements in thyroid therapy guidelines, policies and procedures at a large scale: across regions, provinces and Canada-wide, and on an international scale.

Not health care advice

The information on this website cannot be considered as medical advice to any single person. We regret that we cannot offer medical advice or respond to individual health concerns other than responding in a very generalized manner in comments under posts.

For individual treatment or diagnosis, consult your health care professional.

For individual peer support and tips from fellow thyroid patients, we recommend the “Thyroid Patients Canada Support” group on Facebook and the online health forum called “Heath Unlocked.”

Personal story / interview / data release

These are the terms upon which we publish your story, interview responses, and any other personal data or personal images to be included with it. Usually the process involves enabling you to review and approve of the final draft prior to its publication and to agree to these terms in writing.

By providing your information to us via Facebook Messenger or email with the express verbal agreement to publish it through us, you agree to the following terms:

“I consent to the use of my data, and I grant to Tania S. Smith and the Canadian Thyroid Patients Campaign (the “Campaign”) and its leaders, licensees and successors the right to copy, reproduce, and use all or a portion of my statements and data, such as guest blog posts, interview responses, stories, narratives, images or photographs, audio or video recordings (the “Personal Story”) for incorporation into the Campaign website thyroidpatients.ca and the Campaign’s social media outlets and publications (the “Work”).

I grant the use of all or a portion of the Personal Story in the Work and any of its derivatives in all forms and media electronic, print and other, including social media and related publications throughout the world and in perpetuity.  I understand that the Campaign cannot control information once it has been shared outside of the Work. I grant the right to use my name or pseudonym in connection with all uses of the Personal Story. After publication, I waive the right to further review or approve use of my Personal Story as incorporated in the Work. I grant the Campaign the right to delete the information at any time, to make minor edits for style or clarity, and to shorten or excerpt material in other Campaign posts or publications.

I agree to release and discharge the Campaign and those that the Campaign is responsible for at law from the responsibility and liability of the content and any claims that may arise regarding the use of the Personal Story, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity or copyright. I acknowledge that I have no ownership rights in the Work. I confirm that this release and discharge shall be binding upon my heirs, executors, administrators and assigns.

The Campaign is not obligated to utilize the rights granted in this Agreement.

I have read and understood this agreement and I am over the age of 18. I know that there are risks and benefits to giving this consent. I know that I can withdraw this consent at any time before the initial publication of my Personal Story in the Work by informing the Campaign in writing. This Agreement expresses the complete understanding of the parties.”
